1. What Is the Super Form?
The Company Super Form is the single electronic application (Section 14, Companies Act 2016) used to incorporate a new company through SSM’s MyCoID 2016 portal. It consolidates the old Form 6 (statutory declaration), Form 48A (director’s consent) and parts of Forms 24, 44 & 49 from the 1965 Act into one digital filing.
Upon approval, SSM issues the Notice of Registration (Section 15) and, on request, the Certificate of Incorporation (Section 17).
